CLIENT FORMS

New clients will be sent the following forms for completion, via email, upon scheduling an appointment.  Should there be a problem with the link and your appointment is soon, please contact our office and a new set of forms will be sent to you. Make sure to fully complete the intake form, and read and sign all accompanying forms.  The therapist cannot legally see you until these forms are completed.
